Alzheimer's Disease or Related Disorders Special Care Disclosure
A "Special care provider" means a provider that advertises, markets, or otherwise promotes itself as providing care or treatment to persons with Alzheimer’s disease or related disorders in a special unit or under a special program. [63 O.S. § 1-879.2c] The Health Facility Systems staff receives and processes disclosure forms.
