Take Charge! Program
Offers No Cost Clinical Breast Exams and Cervical Screening

The Durant Family Medicine Clinic, located at 1600 W. University Blvd., Durant, now offers the Take Charge! Program. Women who qualify for the Take Charge! Program will receive no cost clinical breast exams, Pap smears, and a coupon for a mammogram. The clinic hours are Monday through Thursday from 9 a.m. to 4:30 p.m. and Friday from 9 a.m. to 4 p.m. To schedule your appointment at the clinic, call 580-924-5500 ext. 245.

In order to qualify for services, a woman must meet the following guidelines:
· low income; and
· not covered by insurance or a high unmet deductible; and
· not covered by Medicaid or Medicare, and
· between the ages of 50-64.

This program is offered through a partnership between the Durant Family Medicine Clinic and the Take Charge! Program, Chronic Disease Service, Oklahoma State Department of Health.
