Issuing and Obtaining Acknowledgement of Receipt of the Privacy Notice

Supplies of the Privacy Notice and revised Consent for Service form will be mailed to each office during the first week in April. At that time, all existing and new clients will receive a copy of the Privacy Notice and sign a new Consent for Service form. The procedure is as follows:

Existing Clients

All clients and all persons consenting for service for minors must receive a copy of the OSDH Privacy Notice at their next visit to an OSDH clinic beginning April 14, 2003. Clinic staff should answer any questions the client may have about their protected health information or their access to their medical records. Questions that cannot be answered by local staff are referred to Mike Ewald, OSDH HIPAA Coordinator at (405) 271-5585.

Once the client has received the Privacy Notice, the clerical person will complete the identifying portions of Consent for Service, ODH Form No. 303C (Revised 03/03). The client will record their name, relationship and other required information on the new Consent and sign the form. Questions regarding the services the client receives are referred to the service provider. The signed form is retained in the Client Record.

New Clients

Beginning April 14, 2003, each new client or consenter at an OSDH clinic will receive the latest version of the OSDH Privacy Notice and sign the Consent for Service form. Clinic staff should answer any questions the client may have about their protected health information or their access to their medical records. Questions that cannot be answered by local staff are referred to Mike Ewald, OSDH HIPAA Coordinator at (405) 271-5585.
Questions regarding the services the client receives are referred to the service provider. The signed form is retained in the Client Record.

Privacy Notice Changes

Any time the Privacy Notice is revised, the revised notice must be posted in a conspicuous place in the client waiting areas. All copies of older versions are to be destroyed upon receipt of the revised statement. Clients may ask for and receive a copy of the Privacy Notice at any time whether or not there has been a revision to the statement the client received previously.
