External Violations Policy

Employees of the Oklahoma State Department of Health (OSDH) who become aware of violations of HIPAA Privacy rules by outside providers or organizations that occur during the delivery of service to OSDH or its clients must report the violation to their supervisor. Supervisors are directed to forward the complaint to the OSDH HIPAA Privacy Officer at (405) 271-5585 or privacyofficer@health.ok.gov .

Employees of the Oklahoma State Department of Health who become aware of violations of HIPAA Privacy rules by outside providers or organizations that are not related to delivery of service to OSDH or its clients, such as when the employee is seeking medical services from a private provider, have the same rights to report the violation as members of the public. These rights include filing a complaint with the provider or organization or filing a complaint directly with the Office of Civil Rights/U.S. Department of Health and Human Services at:
