Vision: "Add Years to Life and Life to Years."

Mission: To partner with public, private, and voluntary agencies to enable
Oklahomans to live increasingly long, healthy, satisfactory lives by:

•  Preventing death and disability from chronic diseases/conditions

•  Screening for early detection of disease

•  Promoting healthy behaviors throughout the life span

Guiding Principles: A multi-disciplinary team of public health professionals who:

•  provide assessment of data

•  plan programs based on evidence

•  plan programs which stimulate community involvement and are population based

•  evaluate performance by outcome measures

•  procure funding through federal and other resources

•  provide technical consultation

Chronic Disease Service Program Components

Surveillance – the science of collecting, editing, and analyzing all types of chronic
disease data used for epidemiologic trends, program planning and evaluation,
and public policy development.

Professional Education – the education of health professionals throughout Oklahoma
on chronic disease programs, issues, or statutes through publication, presentation, and
skills building.

Public Education – the production, publication, provision and/or presentation of
brochures, community forums, videos, and press releases on topics of chronic disease
or chronic conditions: including breast, cervical, skin, lung, and colo-rectal cancers; diabetes;
heart disease; stroke; arthritis; osteoporosis; urinary incontinence; alzheimer's disease;
physical activity; obesity; and aging.

Health System Change – the partnerships of Chronic Disease Services with advisory
groups, coalitions, and other agencies address policies, standards, opportunities, resources,
and/or barriers to assure accessible, acceptable and affordable quality chronic disease health
care for all Oklahoma.
