Birth and Death Submission Training

These trainings are designed to provide physicians, hospital personnel, funeral directors, and others with instructions for completing and filing a paper copy of the Oklahoma Certificate of Birth, Death or stillbirth. 

Funeral Directors Completing the Oklahoma Certificate of Death

This training is designed to provide funeral directors with instructions for completing and filing the Oklahoma Certificate of Death. By State Law, 63 OS 1-317(b), the funeral director is responsible for filing an accurate and complete death certificate.

Doctors Completing the Oklahoma Certificate of Death

This training is designed to provide doctors with instructions for completing and filing the Oklahoma Certificate of Death. According to State Law, 63 OS 1-317, a death certificate is to be filed with the State within three days of the event. It further states that funeral directors have 24 hours after the death to fill out the Personal Information portion of the certificate and then deliver it to the attending physician. The physician then has 48 hours after the death to fill out the Medical Information portion and return the certificate to the funeral director. The funeral director files the certificate with the State Registrar.

This training is designed to provide funeral directors and doctors with instructions for completing and filing the Oklahoma Certificate of Stillbirth. By State Law, 63 OS 1-318(b), the funeral director is responsible for filing an accurate and complete stillbirth certificate.

Birth Clerks Completing a Paper Copy of the Oklahoma Certificate of Live Birth

This training is designed to provide physicians, hospital personnel, and others with instructions for completing and filing a paper copy of the Oklahoma Certificate of Live Birth. However, facilities using the Windows Electronic Birth Certificate (WinEBC) software to file birth certificates electronically may still benefit from this material, particularly at times when their computer systems may be down.
