Annual Cooperative Hospital Survey

The annual hospital survey is the State’s only comprehensive resource for aggregated hospital data including utilization, personnel, revenue, expenses, managed care contracts, community health indicators, physician models, and much more. The survey includes questions about organizational structure; facilities and services; community orientation and total facility beds, utilization, finances and staffing; patient origin by county of residence; supplemental information and instructions for completing the survey.

Prior to 2009, the survey was a cooperative effort among the Oklahoma State Department of Health, Oklahoma Hospital Association and the American Hospital Association and administered by the Oklahoma State Department of Health, Health Care Information division.  Beginning in 2009, the Oklahoma Hospital Association assumed administration of the survey. 

Data Products/Reports

Survey Data - For surveys collected prior to 2009, the entire survey data are available to the public upon request. The report can be requested using the custom data request form below.

Patient Origin Report – This report is a summary of the information included in the survey for all hospitals prior to 2009. It includes summary on migration of patients between counties, hospital utilization, facilities and services provided and also a report on long term care. The report can be requested using the custom data request form below.
