Family Planning Clinic Services

Pregnancy tests are available on a walk-in basis, Monday-Thursday, 8:00-3:30pm. Family planning exams are available by appointment only; please call the Norman office at (405) 321-4048 or the Moore office at (405) 794-1591 to schedule an appointment.

The following services are provided for women of childbearing age:

Pregnancy test
Pap smear
Breast exam
Testing for Sexually Transmitted Diseases
Testing for anemia
Screening for sickle cell anemia
Blood pressure check
Height
Weight
Education and counseling on methods to prevent pregnancy
Counseling and referrals

The following services are provided for men of childbearing age:

Physical examination
Instructions in testicular self-exams
Examination of the prostate, as appropriate
Testing for sexually transmitted diseases
Education and counseling on methods to prevent pregnancy
Counseling and referrals

Adolescents are encouraged to involve their parents.

Fees- A sliding scale is used. Fees are based on income. No one is denied services based on inability to pay.

Effective September 15, 2009 proof of income will be required for all Family Planning services. Please bring proof of income on your next visit.

Exceptions: Current on Medicaid(SoonerCare/SoonerPlan); Current WIC client, or 100% to pay.

Acceptable forms of income are:

2 pay stubs for everyone working in the household (within the last 90 days)
Recent LES (leave and earnings statement)
Unemployment letter/notice
Current disability assistance(SSI) document
Written statement of financial support
Signed letter from employer stating gross income.
Letter from DHS showing current eligibility of TANF or Food Stamps

If proof is not provided within 30 days, charges will be assessed at 100%

Clients who are 19 and older, with no insurance may qualify for insurance through SoonerCare or SoonerPlan. Please bring proof of identity (photo ID) and proof of citizenship(birth certificate or voter registration card) at your next visit.

Q: What forms of birth control do you provide? A: Birth control pills, depo-provera injection, ortho-evra patch, intra-uterine device, diaphragms, naturalfamily planning & abstinence counseling, condoms, and emergency contraception. Q: How much does it cost? A: A sliding scale is used. Fees are based on income. No one is denied services based on inability to pay. Q: Who is eligible for Family Planning? A: Any women of childbearing age or any men that would like to prevent pregnancy are eligible to receive birth control through Family Planning. Q: Do my parents need to be with me for my visit? A: Adolescents are encouraged to involve their parents. Q: Can I get a pregnancy test at the health department? A: Yes, we offer pregnancy testing. We provide proof of pregnancy letters for Medicaid and make appropriate refferals for clients.
