Adolescents are encouraged to involve their parents. Fees- A sliding scale is used. Fees are based on income. No one is denied services based on inability to pay. Effective September 15, 2009 proof of income will be required for all Family Planning services. Please bring proof of income on your next visit. Exceptions: Current on Medicaid(SoonerCare/SoonerPlan); Current WIC client, or 100% to pay. Acceptable forms of income are:
If proof is not provided within 30 days, charges will be assessed at 100% Clients who are 19 and older, with no insurance may qualify for insurance through SoonerCare or SoonerPlan. Please bring proof of identity (photo ID) and proof of citizenship(birth certificate or voter registration card) at your next visit. If you have any questions please contact us at (405) 321-4048. |
Q: What forms of birth control do you provide?
A: Birth control pills, depo-provera injection, ortho-evra patch, intra-uterine device, diaphragms, naturalfamily planning & abstinence counseling, condoms, and emergency contraception. Q: How much does it cost? A: A sliding scale is used. Fees are based on income. No one is denied services based on inability to pay. Q: Who is eligible for Family Planning? A: Any women of childbearing age or any men that would like to prevent pregnancy are eligible to receive birth control through Family Planning. Q: Do my parents need to be with me for my visit? A: Adolescents are encouraged to involve their parents. Q: Can I get a pregnancy test at the health department? A: Yes, we offer pregnancy testing. We provide proof of pregnancy letters for Medicaid and make appropriate refferals for clients. |
