Beginning on March 1st 2013, we will be conducting an online Themes and Strengths and Forces of Change Assessment. This is another piece of the information we need to collect in order to develop a comprehensive Community Health Improvement Plan (CHIP) for Carter County. The idea of the survey is to provide an opportunity for Resident and Community Leader input and ideas on important issues regarding the status and direction of health care in Carter County.
