Applying to be an authorized WIC Farmers Market Vendor:

• Review Vendor Handbook (all sections) and Sample WIC Farmers Agreement.
• Complete New Farmers Application form (entire form must be completed).

Note: All Farmer Applicants must be a member of an Oklahoma Farmers' Market that is registered with the Oklahoma Department of Agriculture, Food and Forestry.

• Mail completed Farmer Application to:

Upon review and approval of your application. You will receive a WIC Farmers Agreement and Stamp Order Form. Both need to be completed and returned to the address listed above (WIC Farmers Agreement must be signed and notarized).
