Family Planning Program- Frequently Asked Questions
Who is eligible to receive services?
Services are available to individuals who require family planning services. A fee will be assessed for services received. Individuals that are private pay will have a fee assessed on a sliding scale that is based on family size and income. Individuals with income at or below 100% of the Federal Poverty Level (FPL) will not be assessed a fee. Services will not be refused based on inability to pay. Medicaid will be billed for individuals who are eligible for family planning services under Medicaid. Individuals may be eligible either through regular Medicaid (SoonerCare) or the Oklahoma Medicaid Family Planning Waiver (SoonerPlan).
How do I apply for SoonerCare or SoonerPlan?
