How to Receive ServicesGroup of People

Family Planning Program

Who can receive services?
Services are available to all individuals, male and female, who request family planning services. A fee will be assessed for services received. However, individuals with income at or below 100% of the Federal Poverty Level (FPL) will not be assessed a fee. 

Services will not be refused based on inability to pay.  SoonerCare and SoonerPlan will be billed for individuals who are eligible for family planning services under these programs. 

How do I apply for SoonerCare or SoonerPlan?

Contact the local or nearest county health department and request services.

Family planning services are located in county health departments throughout the state of Oklahoma. For information about available services in your area, contact your local county health department or go to the county health department directory. Maternity / Prenatal Care

Who is eligible to receive services?

Services are targeted for pregnant women who are uninsured or underinsured for these health care services or who live where access to prenatal care is limited.
