Fifth Grade SurveyTeacher in Classroom with Children

The Fifth Grade Health Survey was developed in 1998 by a multi-disciplinary team within the Oklahoma State Department of Health under the direction of Maternal and Child Health Service staff. This comprehensive questionnaire includes questions relating to:

1. Access to health care (including insurance coverage).

2. Diagnosis with certain conditions (including asthma and attention deficit disorder). 

3. Behaviors related to alcohol, drugs, or tobacco use. 

4. Household demographics (density, family income, occupants of house).

5. Safety (including helmet usage, access to firearms, and injury history).

This survey is conducted by systematic random sample of 5th grade classrooms in Oklahoma. The student's parent or guardian is asked to complete the survey. The Fifth Grade Health Survey was piloted in 1998 and again in 2001. It’s anticipated surveillance schedule will be every odd school year.
