For Participating Practices

For practices that are participating in the developmental screening initiative, one of the requirements is to complete an online post-implementation questionnaire after 3 months of implementation in the office.  This feedback will provide valuable information to the partners supporting the project.  A staff member from the Child Guidance office will provide a reminder call when it is time to complete this questionnaire.
