ALL licenses and permits expire December 31st, 2015. Should you choose to not renew online, you may print the below renewal application and mail it to the Board Office with a check or money order. No cash will be accepted.

REMINDER: All Continuing Education is required to be reported no later than June 30, 2016. Dentists have a 60 hour requirement and Dental Hygienists have a 30 hour requirement. Click HERE for a PDF file that will give you step by step instructions on how to report your Continuing Education hours online.
To change your address, phone number, email address, or employment information, click HERE to enter the online system. Once you change successfully in this system, we will get notified the following business day and change it in our internal database. Please note you cannot change your name in the online system.
