**ANY ASSOCIATION OR ANY CHIROPRACTIC COLLEGE APPLYING FOR CONTINUING EDUCATION SHALL SUBMIT A FEE OF $200.00 FOR EACH INDIVIDUAL COURSE/SEMINAR.** 

** IF THE COURSE HAS MULTIPLE MODULES AND/OR DATES THEY ARE CONSIDERED INDIVIDUALLY.  THEREFORE, EACH MODULE OR COURSE WITH MULTIPLE DATES WILL BE $200.00 EACH.**

**ALL CONTINUING EDUCATION APPLICATION SHALL BE SUBMITTED 90 DAYS PRIOR TO THE COURSE/SEMINAR BEING ADMINISTERED.**

**ALL APPLICATIONS THAT DO NOT HAVE APPROPRIATE FEES, DOCUMENTATION, OR ARE NOT FILLED OUT PROPERLY WILL NOT BE ACCEPTED!**

**STATUTES AND RULES REGARDING CONTINUING EDUCATION APPLICATION CAN BE FOUND IN TITLE 59 O.S. 161.10a STATUTES AND OAC 140:10-5-2 RULES.**
