To request a Certified Document for the Oklahoma Board of Tests for Alcohol and Drug Influence please fill out the form below and press the submit button. Your request will be filed via E-mail. You can request one certified document at a time (if you need more than one simply return to this page after your previous request is sent.) Each document costs $4.25. Once your request has been received you will be contacted by the Board of Tests regarding your request and payment. If this format doesn't meet your request you can also mail a written request to the Board at PO Box 11415, Oklahoma City, Ok 73136 or fax 405-425-2490.
