Please call the Oklahoma Board of Tests for Alcohol and Drug Influence at 405-425-2460 between 8 a.m. and 5 p.m. CST to retrieve the password required to access the Refusal/Blood Affidavit password required to access the online form.
Once you have the required password, use the "Tab" key to move across the form fields. Once the form is complete, please insert INTOXILYZER PAPER into your printer and set your left and right margin to .25 (1/4 inch) and top and bottom margin to .5 (1/2 inch) and print the page. Then hit the "Submit Form" button at the bottom of the page.
