What do I do with the four copies of the Affidavit?
Serve the Arrestee immediately, give one to the DA, turn in one to your agency if required, mail one to Driver Improvement, notarized if required. DO NOT send duplicate Affidavits to D.I.. If you mail an Affidavit to D.I. make sure your agency does not send in the copy that you turn in.


The Intoxilyzer only prints two Affidavits, this will be in effect in the future.
This is to eliminate duplicate copies being sent to Driver Improvement. Serve one copy to the arrestee and send the other to Driver Improvement, notarized if section 5 is present. If you need additional copies for your agency or the DA make photocopies of the front of the Affidavit and distribute appropriately.


What is the operating procedure, how do I know I followed it, and that it functioned correctly?
The operating procedure is no longer dictated on a color-coded card. The operating procedure is to push the green button and follow the prompts. The breath testing sequence is the same nine (9) step sequence, the enhanced software adds the ability to complete the Officer's Affidavit and print it at the successful completion of the breath test sequence. During the breath test you need to watch the digital display to verify the successful completion of the nine (9) step sequence. The enhanced software has safeguards built in to it that will abort the test if it detects anything wrong with the Intoxilyzer, the simulator control test, the subject's breath sample, or one of the blank analyses. When you get a printed Breath Affidavit the Intoxilyzer functioned correctly and the operating procedure was followed.


What if the subject gives false information, ie false name?
If you still have the Affidavit just write in the correct information and initial the correction. If you have already sent the Affidavit in to Driver Improvement (405 425 2059) you will need to contact them as soon as possible with the correct information.


What if the Affidavit is not readable, the toner cartridge is low or the Affidavit was printed on the wrong side of the Intox paper?
If the toner cartridge is low take it out and shake it a few times and replace it, then press F1 on the keyboard to reprint.
If the Affidavit is printed on the wrong side of the paper remove the Intox paper from the printer and place it back top in first with watermark side up then press F1 to reprint.


What if the subject is a Jr. or II?
Add any suffix in the LAST NAME field. Sample Jr. or Foreman VIII


What if the subject doesn't have a middle name?
Hit the space bar then enter.


What if the subject doesn't have a DL number?
Just hit the ENTER key and it should bypass all of the DL information prompts.


What if the subject requests a blood test after I have already printed the Affidavit?
Write in the blank space at the bottom of the Affidavit "SUBJECT'S REQUEST, BLOODKIT NUMBER (write in the blood kit number)"


What if I want a blood test after I have already printed the Affidavit, the breath test results are low and I suspect the subject is under the influence of drugs?
If you have conducted a breath test and completed the Affidavit before you requested a blood test you can push the green button again and produce a Blood Affidavit or use the preprinted Blood Test Officer's Affidavit. Put a notarized Affidavit and a copy of the breath test in the blood kit and mail to OSBI.


The keyboard stopped working.
Hit the shift key and it should continue typing. If that does not work then you will have to turn off the Intoxilyzer, wait about 10 seconds, and then turn it back on.


I can't type in the telephone number after I backspaced to make a correction.
You will have to backspace to the beginning of the PHONE NUMBER field and retype the whole number.


What if the Simulator temperature is too high or too low?
The control test will be out of tolerance and your test will be invalid, notify the Intoxilyzer Specialist of the problem. You will have to do a blood test instead.


What if the Intoxilyzer is DISABLED and I need to do a Blood Affidavit or a Refusal Affidavit?
If there is another Intoxilyzer available, close to your location, go there and produce an Affidavit. If you do not have another Intoxilyzer available notify the Intox Specialist to have them put the instrument back in service.


The DATE field won't let go back more than one day.
If you are completing a Blood of Refusal Affidavit more than one day after the arrest you will have to put in the current day that you are completing the Affidavit and in the REAS GND NARR field put in the actual date of the arrest along with the narrative.


What do I have to give the subject if I go to the hospital first and do a blood test?
Fill out the Consent to Blood Test form or the preprinted BLOOD TEST OFFICER'S AFFIDAVIT, make a copy front and back and give it to the subject. You do not have to return to the hospital to give the subject ANYTHING ELSE.


What date do I put in for Serving of Notice on a Blood Affidavit?
You do not have to fill out the Serving of Notice on a Blood Affidavit. You can put in the date that you gave the subject the Consent to Blood Test form if you want to.


Do I still have to send in a Log Of Tests And Maintenance Record with the new software?
When you change the solution you will still need to send in the Log Of Tests within 10 days of the solution change. When you print the Log use regular 8.5 x 11 paper or turn the Intoxilyzer paper around so the bottom is in first, this will print the Log on the clear portion of the paper so it is readable.


If I use the new BLOOD TEST OFFICER'S AFFIDAVIT do I have to go to the Intoxilyzer to do a Blood Affidavit.
No, the new BLOOD TEST OFFICER'S AFFIDAVIT has a notary section on it. The key point to remember is "If the blood test is for the State (officer requested)there must be a sworn (notarized) statement sent with the blood kit."


Is there a BLOOD AFFIDAVIT and a REFUSAL AFFIDAVIT I can use without going to the Intoxilyzer?
There is a BLOOD AFFIDAVIT and a REFUSAL AFFIDAVIT at the end of the Training Bulletin. You can print them out to have copies onhand. These two new Affidavits have a new section 5 which does not require a notary. ANYTHING PRODUCED ON THE INTOXILYZER STILL HAS TO BE NOTARIZED. READ SECTION 5, if there isn't a place for the notary to put in thier information then the arresting officer will complete section 5 and send in the completed form as required.
