Open Records Requests
In order to ensure that the correct records are sent in a timely manner pursuant to the Oklahoma Open Records Act, Title 51 of the Oklahoma Statutes, please include the following information in your request:
- Serial number of the device,
- Name of the tested individual,
- Agency Administering the test, and
- Date of test.
An Open Records Request form is available for your convenience. An invoice in the amount of $0.25 per page for uncertified copies and $1.00 per page for certified copies will accompany your documents.
