Registration cost:  $110.00           Registration is limited and includes lunch.

Scholarships are available for parents who demonstrate financial need.

Registration:  To register, fill out this form and submit (mail or fax) to the address below.  Purchase Orders are accepted from school districts and agencies. All registrations must be in writing and fully paid (or reserved by purchase order) prior to Monday, November 10, 2008 for any participant to attend.  If a registered attendee cannot attend, the registration may be transferred to another name.  All purchase order reservations must be paid even though the registered attendee (or approved substitute) does not attend. 

Cancellation.  If written cancellation notice is received in this office prior to Monday, November 10, 2008, a full refund will be made.  Refunds will not be made under any other circumstance.  Any registered person may transfer the name of the registration to another person through this office.

Payment is enclosed.  All registrations must be pre-paid, unless reserved with a purchase order.  No registrations will be taken at the door without prior authorization by this office.

Purchase order is enclosed or faxed (918-712-9058).  All Purchase Orders must be received  in this office by Monday, November 10, 2008 and signed by official school district or agency personnel.
