What if my Mailing Address is not my Street Address?
For Street Address, fill in the address people use to drive to where you live. For Mailing Address, fill in the address people use to mail items to where you live. If the addresses are the same, do not fill in anything for Mailing Address.
Who is a Contact Person?
We contact a Contact Person only to schedule the smoke alarm installation. A Contact Person can be a friend, family member, or anyone who has frequent contact with you. If you are not home sometimes between 8:00am and 5:00pm, you should fill in information for Contact Person. If you have trouble communicating with people over the phone, you should fill in information for Contact Person.
What are the differences among Applicant, Preparer, and Contact Person?
Your application may have three different names within it: 1) the Applicant 2) the Preparer, and 3) the Contact Person. The Preparer is the person filling out the form for the Applicant. For example, the Preparer could be a Deaf-Blind Specialist or a school counselor. But, the Applicant may need a different person to be the main contact. This Contact Person can be a friend, family member, or anyone who has frequent contact with the Applicant. The Contact Person helps make sure that we can reach someone to set a date for the installation.
What are the types of residence?
One family means only one family lives in the residence and the residence does not share walls with other residences. Multi-family means that more than one family lives within the building. A duplex and a triplex are types of a multi-family residence. If there are more than four residences within the same building, it is an apartment.
What if I have people visiting, but not living, in my household?
Fill in the number of people who normally live in the household. If you have temporary guests, do not count them in the number of people. If the guests are staying more than a few weeks, count them as people living in the household.
Who pays for an interpreter if I need one during the installation?
We will pay for an interpreter if you need one when the installer comes to your home.
