After a SoonerStart assessment, please complete the "Assistive Technology Devices Use for Assessment "Form. There are two form options: 

1) Please fill out and submit the Online Form: AT Devices Used for Assessment. 

Do Not Submit via mail or email if you submit form online.   

2) or, fill out the AT Device Use form PDF and save, print, or email to Oklahoma ABLE Tech.
