There are two form options: 

1) Online Form: AT Devices Used for Assessment or 

2) or, fill out the Assistive Technology Devices Used for Assessment PDF.  Save and email it to tessa.stinnett@okstate.edu or print and fax to Oklahoma ABLE Tech at 405-744-2487. 

Do not use method 2 if you submit the online form.
