HB 1304, also known as the "Information Technology Consolidation and Coordination Act," was signed into law by Gov. Mary Fallin in May 2011. To comply with the legislation, OSF's Information Services Division needs the following agency information technology and telecommunications functions by Sept. 1. the information should be submitted to isdexecutive@osf.ok.gov.

The template is divided into the following sections:
The summary by object code and object code detail are populated when the other tabs/sections are filled in. The only field you need to complete on the object code summary is the blue field in the upper left-hand corner (Row 2 / Column B & C). This is where you will indicate the Agency/Department/Function name. If you are only completing a template for the total agency’s information technology function, insert "Total Agency" in the blue field.
On the payroll costs by month sheets, only the yellow fields need to be completed. The remainder of the template will automatically be populated. All other columns are automatically calculated based on the information provided in the yellow fields. Please enter a 4 in the benefits field. The salary page allows for 40 employees to be entered. If you need more fields, please contact the helpdesk and we can provide assistance.
On the professional services sheet, the blue fields need to be completed. The yellow fields (expense description) will automatically populate.
On the travel expense sheet(s), the blue fields need to be completed. The totals will automatically populate.
On the administrative expense sheet(s), the blue fields need to be completed. The yellow fields (expense description and totals) will automatically be populated.
On the furniture, equipment, etc. expense sheet(s), the blue fields need to be completed. The yellow fields (expense description and totals) will automatically be populated.
On the miscellaneous expense sheet(s), the blue fields need to be completed. The yellow fields (expense description and totals) will automatically be populated.
